The Resident Navigation Specialist guides participants into appropriate programs, working closely with eligible residents to navigate multiple benefits programs both internally and externally. Comprehensive assessments to identify strengths and barriers are conducted to assist residents in achieving their goals and move towards self-sufficiency. We monitor and evaluate service delivery in order to modify action plans as needed.
