Programs and Services
The Case Manager serves as a guide into programs and services within and beyond CAPP. We assist income eligible residents achieve their goals and move towards self-sufficiency through:
- Comprehensive assessments that identify individual strengths and barriers
- Internal and external referrals
- Continuous monitoring and evaluation of service delivery
- Modification of action plans, as needed
